Guidance for Applicants

67.65 (b)(1)

  • If you have lost your medical certificate, it was stolen or it has been destroyed without recovery; you are required to complete the Application for Replacement of Medical Certificate - All Sections

67.65 (b)(2)

  • If you have accidentally damaged your certificate (it went through the wash, the dog chewed it etc) and still have it, you are required to complete the Application for Replacement of Medical Certificate - Sections A, B, and D. You must forward the damaged certificate to the medical unit with your completed application.

Remember to enclose payment or complete your payment method or your application will not be processed and it will be returned to you.

What is a Statutory Declaration?

A Statutory Declaration is a written statement of fact that is signed in the presence of a solicitor or a Commissioner of Oaths.
